How is a microdermabrasion performed?

The procedure is usually well-tolerated without anesthetic. If needed, the skin can be pre-treated with an anesthetic cream. The level of the treatments is gradually increased over six treatments, as tolerated, to achieve optimal results. Microdermabrasion is also known by other names such as Parisian Peel or Megapeel.  At the Laser & Skin Surgery Center, microdermabrasion is performed with a medical unit known as the Mega Peel. The medical unit allows a higher level of treatment and the ability to enhance the results patients may have experienced at non-medical facilities.

Benefits of microdermabrasion

The benefits of microdermabrasion are a result of the combined effect of the crystals, as well as the vacuum pressure, which removes  dead skin cells from the skin’s surface and pores. The vacuum enhances the results obtained by crystals alone (in microdermabrasion creams or kits).

How long is recovery after a microdermabrasion?

Little to no recovery time is necessary after a microdermabrasion treatment. This is a minimally invasive treatment and does not require any anesthetic. You may experience some minor redness or discomfort after each session, but should feel well enough to continue your daily routine as planned.

When will I notice the results of my microdermabrasion?

A series of treatments, usually six, will result in a gradual improvement in the texture and overall appearance of your skin. The benefits of microdermabrasion are a result of changes in the epidermal and dermal layers of the skin.  Maintenance treatments every six weeks (*), along with a planned skin care regimen will help to keep your skin in optimal condition.

Brown Spots / Discoloration

IPL stands for Intense pulsed light. Over time, brown spots may appear on sun-exposed areas of the skin. They are often called “age spots” or “liver spots.” The face, neck, upper chest, arms, and legs can be affected as a result of years of previous sun exposure. Those areas that have greater sun exposure have more spots (e.g. often on the left side of the face, especially the cheek bone.) Fortunately, these spots can be successfully treated in our office with the state-of-the-art laser and IPL. Other treatments include chemical peels and the Nu-Derm® skin care system. All treatment options will be explained in deciding which treatment is most suitable for the individual patient. Sun-protection is important to prevent the appearance of new brown spots.

Dark Circles

Dark circles under the eyes can be a challenging problem to hide with makeup. There are different reasons for the appearance of dark circles. Increased pigmentation of the skin under the eyes is one reason for the appearance of dark circles. Another reason is the appearance of “bags” under the eyes. A third reason is a loss of fullness of the upper cheek immediately below the eyes. Given the different reasons for dark circles, the correct treatment or combination of treatments must be determined for each individual. The treatment options include the use of lightening agents, laser treatment, fillers to correct the loss of fullness and surgery for “bags” under the eyes. Any of these treatments can be performed safely in our office. A consultation with Dr. Diwan will help determine the cause(s) of the problem in your case and define the best treatments for you.

Double Chin

The appearance of a “double chin” creates an older and unattractive appearance. The presence of excess fat under the chin leads to the “double chin.” Liposuction, in the office, with local anesthetic, is a simple, quick and safe solution for this contour problem. If there is significant amount of sagging skin, a neck lift may be a more suitable treatment for removal of the fat and the loose skin. Kybella®, a recently FDA-approved treatment, is injected just under the skin to destroy fat cells and reduce the double chin fat under the skin. It will soon be available at our office.

Growths, moles, and skin tags

The appearance of various kinds of skin growths on the skin is a common occurrence. Many benign (non-cancerous) growths appear over time and are more common in the elderly. Seborrheic keratosis, moles, angiomas (red-purple bumps), enlarged oil glands and skin tags are most frequently encountered. Depending on the nature of a particular growth, it can be removed in the office with quick minor surgery (a local anesthetic–numbing injection–will be used) or  treated with a laser, or burned off with an electric needle ( with local anesthetic). Each option provides excellent cosmetic results.

Hair (Excess or Unwanted)

Being overly hairy can be an embarrassing physical trait for men and women. Often inherited, excess hair growth may appear on the face, back, chest, arms or legs. Sometimes, though, excessive facial hair growth in women can be a sign of underlying hormonal disorders which must be recognized and treated. Some drugs, such as Cyclosporine, can also cause increased body or facial hair. A thorough history and examination help to determine if there is an underlying cause for the excessive hair growth, which can then be treated appropriately.

Hollowing

Hollowing of the face or loss of facial volume occurs naturally over time, as part of the changes associated with aging. Aging affects the thickness of the skin as well as the tissues underneath the skin. Loss of volume and support underneath the skin leads to loss of fullness of the cheeks, temples and the sides of the chin that are associated with youthfulness. Sunken cheeks, creating a gaunt appearance, contribute significantly to “looking older.”  Restoration of lost volume in the face is an important component of rejuvenation and helps to optimize the results from other treatments. Juvederm Voluma®, FDA -approved and utilized by Dr. Diwan since 2014, immediately restores youthful contours of the cheeks and chin. Results usually vary by individual and achieved by full correction can last for 18 to 24 months. Another effective and safe way to restore facial fullness is with Sculptra®. Sculptra is a synthetic injectable product which gradually stimulates natural augmentation by one’s own tissues. Treatments are continued until the desired result is obtained. Restoring fullness of the face also helps to take up the slackness in loose skin. The results obtained from Sculptra can last for two to three years. Periodic touch-ups help to keep up with ongoing aging. Loss of volume also affects the hands which start to look more thin and bony over time. These changes can also be treated with Sculptra. Besides being used to restore a loss of volume associated with aging, Sculptra can also be used to correct depressions resulting from liposuction, or other injuries to the deeper tissues under the skin.

Melasma

The term “melasma” refers to the patchy brown pigmentation that appears on the face, commonly associated with pregnancy or the use of birth control pills. It is also referred to as “pregnancy mask.” The pigmentation most often involves the forehead, cheeks, and upper lip and, typically, worsens in the summer. Most women have tried over-the-counter and prescription bleaching creams and chemical peels without much success. The treatment of melasma is more effective with a specially formulated combination of prescription-strength topical lotions that can be purchased in our office. Specially formulated creams can be ordered for stubborn melasma that doesn’t respond adequately to the regular prescription-strength lighteners. Laser treatment, with a fractionated laser, can be added, if necessary. The reappearance of melasma, after it resolves, must be prevented by using effective sun-protection measures. Perspiration (Excessive)

Excessive perspiration of the underarms, hands and/or soles of feet can be an embarrassing condition. If the problem cannot be controlled with prescription antiperspirants, Botox® is a very effective alternative that can significantly reduce perspiration for several months, as long as a year or more,  at a time. Botox is injected directly in the skin, in the problem areas, with minimal discomfort, providing  great relief from an otherwise difficult problem.

Pores (Enlarged)

Skin pores are present everywhere on the skin and their appearance is a unique feature of one’s skin. Pores, which are not apparent until adulthood, become more visible on the face with continued enlargement of the oil glands in the skin. The appearance of pores can usually be improved by exfoliating with topical agents, superficial chemical peels or microdermabrasion. Continuing a maintenance treatment program helps to prolong the benefits.

Redness

Redness of the skin may affect the face and/or the neck. Redness may be a result of sun-damage, Rosacea, genetic inheritance or a combination of these factors. The sides and front of the neck, the cheeks, nose, chin and the earlobes can be affected. Redness can be improved with state-of-the-art laser treatments in our office. For some patients, small doses of Botox may be helpful. Treatment of the underlying cause of the redness and avoidance of triggers, if possible, is important.

Rhinophyma

Rhinophyma is a deformity of the nose that most commonly affects middle-aged men. Gradual overgrowth of the oil glands in the skin causes a bumpy enlargement of the lower half of the nose. The deformity may become so severe that a patient may curtail his social activities and become confined to his home. Fortunately, the disfigurement can be treated, by sculpting the nose back to a more normal shape and size, with a carbon dioxide laser. The treatment is performed in our accredited surgical office with local anesthetic (numbing injection). Healing usually takes two weeks and pinkness fades over two to three months. Sun protection is very important for at least six weeks after healing.

Scars

Scars are left in the skin as a consequence of injury. The kinds of skin injury that leave scars include conditions like acne and chicken pox; surgery for removal of moles, cancers and other growths, and accidents. Scars can have different appearances, even when caused by the same type of injury,  and not all injuries result in scars. The deeper the injury the more likely it is to leave a scar. Scars that are visible may be of cosmetic concern. Different options are available for treatment of scars, including removal of scars with special surgical techniques, laser resurfacing, and fillers for the treatment of depressed scars. A combination of treatments may   optimize the cosmetic result.

Veins

“Broken” blood vessels gradually appear on the face as a result of sun-exposure, Rosacea, heredity, or a combination of these factors. Patients are sometimes concerned about giving the impression of being heavy drinkers. Camouflage of the blood vessels with make-up becomes increasingly time-consuming and difficult. These blood vessels can be significantly improved, if not completely removed, by non-invasive treatments with the latest laser technology . The state-of-the-art laser used in the office  selectively targets the blood vessels in the skin without breaking the skin surface. The treatment is followed by mild swelling and pinkness for 1-2 days. The blood vessels gradually improve resulting in significant improvement after a single treatment. Further improvement is achieved with additional treatments, which are spaced 6 weeks apart. With the blood vessels removed, the original appearance of the skin without “broken” blood vessels is restored.

The easiest way to judge a person’s age is by the presence of wrinkles. Treatment of wrinkles is, therefore, an essential aspect of rejuvenation and is frequently requested at our office. We offer a variety of simple options to effectively reverse wrinkles.

Combination Treatments

As aging affects different aspects of your face, Dr. Renuka Diwan MD, FAAD will often recommend combining different injectable and filler treatments with other non-surgical treatments to address each area of concern and achieve the most improvement. A combination of non-surgical treatments can help you completely rejuvenate your look without having to undergo surgery and provide better results than anyone treatment alone.

How are combination treatments performed?

An example of Dr. Diwan’s strategy of combination treatments would be Botox® for treating frown lines, Restylane® or Juvederm® for smoothing away smile lines, and a laser treatment to improve the appearance of broken blood vessels (visible veins).

Another common example would be the treatment of deep frown lines with Botox plus a filler (Restylane, Juvederm, or Radiesse) to achieve the best result. This combination of non-surgical injectables and fillers is often referred to as a liquid facelift because it provides patients with a subtle lift without requiring any surgery or downtime.

How long does it take to recover from a non-surgical combination treatment?

Each of these treatments used by Dr. Diwan has a very limited recovery time. In some cases, none at all. Patients can return to work immediately after their treatment.

What to expect from your results following a liquid facelift

Each individual treatment will achieve results at different rates. While some treatments (such as fillers) will provide you with immediate benefits, others may take a bit more time for you to notice results.  With Botox and some laser treatments, for instance, you will likely see an immediate change in your appearance with gradual improvements over the next few weeks.

Sclerotherapy in Westlake, OH

Spider veins—the thin visible dark blue or red veins that appear on the legs—are a common problem, especially among women. While they usually do not cause symptoms, spider veins can affect your confidence and may prevent you from wearing certain types of clothing due to embarrassment. Thankfully, however, there is a way to treat these veins, which helps to boost confidence for many patients!

How is Sclerotherapy performed?

Sclerotherapy remains the treatment of choice for  patients with spider veins, and can provide extremely effective results. The results are more predictable than those from laser treatment of spider veins.

Sclerotherapy is an office procedure, which involves injection of the spider veins with a concentrated salt-water (saline) solution. Other solutions are available for injection, but saline is the safest and most effective in our experience. Dr. Diwan has helped many men and women by safely and effectively treating their spider veins with saline injection sclerotherapy.

Sclerotherapy causes minor discomfort—similar to that from eyebrow tweezing —for most patients. If desired, Dr. Diwan can use an anesthetic cream prior to your Sclerotherapy to minimize any discomfort.

How long is Sclerotherapy recovery?

Following your treatment, you should be able to return to your normal routine with little to no downtime. You will have small bruises at the injection sites which fade over 7-10 days. Wearing long pants or a long skirt helps to hide the bruises. We recommend wearing compression hose for the treated areas for three days. Dr. Diwan will provide you with detailed care instructions once your treatment is complete.

When will I notice the results of my Sclerotherapy treatment?

After just one sclerotherapy treatment, the problem veins often fade dramatically or disappear completely. However, it will take six weeks (*) for the final result from the treatment session to be evident.

A second or third treatment may be recommended for further improvement and disappearance of the veins. The best time to have sclerotherapy is in the fall, winter, and spring since it is easier to keep the legs protected from the sun and hide the small temporary bruises.  Sun protection before and after sclerotherapy is important to minimize the risk of pigmentation, especially in darker skin types.

As new veins may appear, over time, in patients with a tendency to have spider veins, additional treatments with Dr. Diwan may be necessary to keep your legs looking their best.

Can Anyone Be a Candidate for Sclerotherapy?

Most patients who receive sclerotherapy are between the ages of 30 – 60, (but younger and older patients can be treated) and are in good overall health. If you are pregnant or breastfeeding, this treatment is not recommended. Sclerotherapy is also not recommended for patients with blood clotting disorders.



There Any Risks With Sclerotherapy?

Just like any other injectable treatment, sclerotherapy has risks. These complications can include infection (which can be treated with antibiotics), pigmentation of the skin, appearance of tiny spider veins and, very rarely, a scar. However, with the proper care and attention these risks can easily be avoided. Dr. Diwan is dedicated to ensuring all patients have a safe and comfortable experience during their sclerotherapy treatment, and achieve excellent results.

How Long Will the Results of My Sclerotherapy Treatment Last?

While the veins that are treated are permanently removed and their appearance eliminated, new spider and varicose veins may develop on the legs over time due to the predisposition, usuallly genetic, to forming them and aging. As new veins appear, periodic treatments with Dr. Diwan may be necessary to keep your legs looking their best.

Lip Augmentation

Thin lips are a common concern for many and may be the result of either genetics or age. Having fuller lips often translates into feeling more attractive, a more youthful look,  higher self-esteem, and of course–more smiles. At the Laser and Skin Surgery Center, Dr. Renuka Diwan MD, FAAD offers patients a non-surgical approach to lip enhancement, helping her patients achieve the fuller, more youthful smiles they desire.


How is a non-surgical lip augmentation performed?

The lips become thinner with age and restoring fullness is an important part of facial rejuvenation. Greater fullness and definition of the upper or lower lips can be achieved with a painless, surgery-free lip enhancement. Dr. Diwan is able to increase the fullness of the lips using dermal fillers such as Restylane®, Restylane Silk  or Juvéderm Ultra®. These fillers are safely injected into the lips to enhance their definition and size while maintaining a natural appearance and proportion. These filler injections can be repeated periodically to maintain the desired fullness of your lips. A very small amount of Botox, carefully injected in the lips, can also increase the visible height of the lip, and help to prevent too much fullness from fillers.

How do Lip Fillers Work?

Most lip filler treatments involve the use of a hyaluronic acid (HA) filler.

HA fillers are injected directly into the lips to enhance definition and/or fullness. HA is beneficial because it not only plumps the lips but also attracts and binds water from within to deliver a healthy, natural-looking pout. To really understand this process, it’s important to first know what HA is. This sugar derivative is produced naturally in the body to hydrate eyes, joints, and skin. The power of this compound is that it can bind to and retain water molecules equal to 1,000 times its own weight. With such moisturizing abilities, HA makes for a perfect filler: it is biologically inspired, protects tissues, and gives long-lasting results.

Do Lip Injections Hurt?

Getting injections into the delicate tissue around the mouth can be intimidating. D. Diwan recommends applying a numbing cream to desensitize the skin before commencing with your treatment. There are other simple techniques used in the office to reduce discomfort as well. Many of the injectable products that are used for lip enhancement contain a little bit of lidocaine, a mild anesthetic which also reduces discomfort. Generally, lip filler injections are very well-tolerated. This is true even when treating thin or deflated lips or fine lines that border the lips. Are Lip Injections Right for Me?

Lip augmentation is a common nonsurgical procedure that is performed nearly every day in our office. However, just because lip injections are popular doesn’t mean it is right for you. How do you know? You can begin to explore the value of lip injections if you aren’t happy with the shape of your mouth. Are your lips naturally thin? Do they disappear when you speak or smile? Have your lips gone from luscious to lackluster as you’ve aged? Do you notice the vertical lines around your mouth more than the volume of your lips themselves? It doesn’t take long to spot the things about your lips that you might not love. Once you do, your next step is to talk to Dr. Diwan, an experienced injector, to find out if nonsurgical lip augmentation is right for you. 

Could Lip Fillers Make Me Look Unnatural?

Our goal is to enhance your look not just your lips. Dr. Diwan has an esthetic eye for keeping the fullness of your lips in proportion to the rest of your face. Unfortunately, people often have overdone lips from less experienced injectors. When you consult with Dr. Diwan you can expect to have a discussion of what will enhance your lips and your face, without looking unnatural. Lip augmentation may take place over a couple of appointments to make sure the enhancement is gradual and not overdone. 

Is there anything I Should Avoid After Lip Injections?

After lip injections, for 24 hours, you may want to observe the following:

  • Apply cold packs and keep your head elevated 
  • Avoid rubbing, massaging, or exfoliating the lips.
  • Kiss, but do so gently!
  • Avoid using a straw when drinking.
  • Drink lots of water and avoid alcohol.


How long is lip augmentation recovery?

There is little to no downtime following a non-surgical lip enhancement with Dr. Diwan. You may experience some minor swelling or discomfort, but this should fade over the next couple days. Dr. Diwan will provide you with detailed care instructions.  Most patients are able to return to their daily routines.

When will I notice my lip enhancement results?

You will notice an immediate change in the look of your lips following your treatment. At first, they may appear a little swollen, but over the next few days, any swelling will dissipate, and your lips will appear naturally fuller.


How long will the results of my lip augmentation last?

Because the fillers used will not provide permanent results, we recommend returning for a little more filler to maintain lip enhancement every 6 months or when you notice that your lips are losing  their fullness (*).

Sunscreen

Sunscreen is an important tool for the protection of the skin from the sun’s ultraviolet rays. Application of sunscreen on sun-exposed parts of the skin, including the lips, is essential for keeping the skin healthy and looking its best—especially for fair types of skin. Effective sunscreens help prevent:
  • Painful sunburns
  • Skin cancers
  • Accelerated aging of the skin (premature wrinkles, brown spots, and facial veins)
  • Cold sores on the lips (more frequent after sun exposure, can be prevented from being activated by use of lip sunscreen)
  • Uneven skin tone with the appearance of darker spots and  patches, especially for darker complexions

Sunscreen SPF

The SPF number on a sunscreen indicates its effectiveness against ultraviolet B (UVB) rays, but not against the (equally important) UVA rays of the sun. Many different sunscreens, with a variety of ingredients, are available. The majority of sunscreens contain chemicals that absorb UVB rays—and to a lesser extent UVA rays—and in doing so are inactivated in two hours or so.  The most effective sunscreens contain zinc oxide and / or titanium dioxide. These ingredients are broad-spectrum being effective against both UVB and UVA and do not become inactivated as they protect the skin. To help you optimize the health of your skin, the best zinc and titanium containing sunscreens, such as TiZo, is available at our office. If you have had skin cancer(s) or want to prevent one, the most effective sun protection should be a part of your daily skin care regimen. The above sunscreens are also gentle enough to protect your skin after healing from corrective laser treatments, microdermabrasion or chemical peels.

Sunscreens with Zinc

Zinc-containing sunscreens available at our office have tiny particles of zinc that do not look white on the skin, have an elegant matte finish, and are applied under make-up or powder. Tinted sunscreens, free of PABA, preservatives, oils and fragrances, gentle enough for daily use on sensitive skin, and suitable for dry, normal and oily skin are also available. Some of the products are water-resistant. A new light TiZo foam sunscreen is now available. Remember to protect your lips with LipTect SPF-45. Be sure to ask for written sun protection guidelines to help you get the most benefit, when you purchase your sunscreen products.

How Often Will I Need Chemical Peels?

Because the skin is continuously replacing itself, we recommend patients undergo superficial chemical peels every month for maintenance. In the span of a month, the skin on your face can go through many changes. Because of this, the skin needs to be treated with a monthly chemical peel to maintain rejuvenation and control acne.

Who Is a Candidate for Microneedling?

Microneedling is a great skin treatment for anyone who is looking to achieve rejuvenation of the skin and improve imperfections. Ideal candidates for microneedling are in overall good health, do not have active acne, are non-smokers, have a good skin care regimen, follow sun protection guideliens and have realistic results in mind. When undergoing microneedling, it’s important to be able to commit to multiple sessions to achieve the best result.

The contraindications for microneedling are those who have active acne, infections, or an underlying health condition that makes it difficult for the skin to heal.

How Long Will It Take to Recover from Microneedling?

Following a microneedling session, patients usually experience mild redness and swelling of the face for the first 24 to 48 hours. After this time, the swelling and redness fade, and most patients return to their normal routines within 24 hours of their treatment. Patients notice improvement in the skin over a few treatments and for a few months after treatments. A little peeling and temporary breakouts may also occur after this treatment, but are merely a sign that your skin is recovering.

You will have a pre-treatment consultation with Dr. Diwan in order to determine the appropriate type of peel based on your unique skin type and condition. Dr. Diwan is trained in effectively treating patients of all different skin types and colors.

Different peeling agents are used for the different conditions mentioned above.

How long is recovery after a chemical peel?

The healing time depends on the type of peel and can range from one hour to two weeks. You should feel well enough to return to work shortly after a superficial peel, but may choose to take a couple days off or work from home if you are experiencing high levels of peeling.
