Skin Cancer Treatment

Basal Cell Cancer is the most common form of skin cancer, followed by squamous cell cancer.  Melanoma is less common than either basal or squamous cell cancer, and it is the most aggressive of skin cancers. In addition to these three types, other less common forms of skin cancer are also encountered.

Basal Cell Cancers

Basal cell cancers can have a variety of different appearances to the naked eye. Under a microscope, basal cell cancers have different growth patterns as well – some being more aggressive than others. Different treatments are available for basal cell cancers. These include:
  • Simple surgical removal followed by a repair of the skin
  • Curettage & electrodessication (scraping & burning)
  • Mohs Micrographic Surgery followed by reconstruction of the skin
  • Treatment with Efudex cream
  • Radiation therapy
Except for radiation (which is performed at a hospital or outpatient radiation therapy facility), all treatments are offered at our office. Which treatment is recommended for a particular skin cancer depends on a number of characteristics of the skin cancer. After considering the location, size, presence or absence of clear definition, the failure of previous treatments, microscopic appearance, etc., the appropriate treatment is recommended.

Mohs Surgery

Mohs micrographic surgery provides the best cure rate for many skin cancers. An office procedure performed with local anesthetic, it involves step by step removal of the cancer until only cancer-free tissue is left, as determined my microscopic examination of the removed tissue. In 99 percent of cases, the wound created in the skin by removal of the skin cancer is evaluated and reconstructed within the following days, with very gratifying cosmetic results.

Skin Cancer Treatment Reconstruction

Reconstruction of wounds in the skin, resulting from skin cancer removal, restores normal appearance and function. Dr. Diwan has fellowship training in Procedural Dermatology which gave her the opportunity to learn reconstructive techniques. She successfully uses these techniques to repair wounds,  paying close attention to aesthetic details. Being a cosmetic surgeon, as well, she applies refined cosmetic techniques to reconstructions, giving our patients the best possible outcome. Dr. Diwan has over 22 years’ experience in reconstructing wounds, the majority being on the face where the cosmetic result is especially important. The scars from reconstruction are created with careful planning to give you the best chance of healing with an inconspicuous scar. All surgical procedures are performed personally by Dr. Diwan. Reconstructions are performed in our surgical office facility with local anesthetic for comfort, safety and quick recovery.

Scar Revision

Scars happen to everyone—whether due to an injury, health condition such as acne, past surgical procedure, or any number of other reasons. However, these scars can act as a reminder of a sensitive subject from the past and often negatively affect a person’s self-esteem. A professionally performed scar revision treatment can help to reduce problem scars, and help men and women feel more confident about their appearance. Dr. Renuka Diwan is specially trained in dramatically reducing the visibility of scars using a variety of different scar revision methods.

How is scar revision performed?

Your scar revision treatment will be completely unique to your specific scar. Dr. Diwan will make a treatment recommendation based on your type of scar, the location of your scar, and the size of your scar. Treatment of scars may include cortisone injections, dermal fillers for elevation of depressed scars, surgical removal and re-direction of an unfavorably oriented scar or depressed scar, fractionated CO2 laser resurfacing, or state-of -the-art laser treatment for fading red scars and smoothing raised scars. A personalized evaluation with Dr. Diwan will help to determine the best treatment(s) for your scar(s) and restore your confidence.

How long is scar revision recovery?

The recovery period following your scar revision will vary based on the treatments used for your procedure. In most cases, you should be able to return to your normal routine immediately after your treatment. Some surgical scar revision procedures may cause some minor discomfort and require a day or two of recovery.

When will I notice the final results of my scar revision?

Because each scar revision is different for every patient, it is difficult to say how long it will take to see your final results without having a consultation with Dr. Diwan. Once Dr. Diwan analyzes the condition of your scar and develops an effective treatment strategy , she will be able to provide you with a more detailed timeline of any necessary recovery period, and tell you when you will notice the final results of your treatment. Patients who have undergone scar revision are often thrilled with their results. Treatment can not only dramatically reduce the appearance of scars, but, in some cases, can even fade them completely.

This tumescent liposuction procedure allows for the safe and painless removal of fat in the office with local anesthesia. A medication can be given to lessen anxiety and help you relax, if needed. A local anesthetic is used to numb the fat for removal  through suction. The tumescent method lessens bleeding, bruising, and post-operative discomfort and speeds recovery. How long is tumescent liposuction recovery?

Tumescent liposuction allows for a quicker recovery period than other forms of liposuction and cosmetic fat reduction. Most patients have minimal bruising.  Swelling of the treated area is normal and will gradually fade over a few weeks. Depending on your job, you should be able to return to work 3 days later. Physical activity must be minimized for a week and gradually increased over six weeks.

When will I notice the final results of my liposuction?

Sagging of the skin on the face is primarily a result of a loss of elasticity and the effects of gravity pulling down on the facial tissues over time. A facelift helps to correct this facial sagging by lifting the tissues upward. How is a Mini Facelift or Neck Lift Performed?

This type of facelift can be performed for healthy patients without the risks of general anesthesia (using local anesthetic) by Dr. Diwan in our office with excellent results. A neck lift can also be performed either in combination with or without a mini facelift, for patients who are interested in correcting sagging of the skin around the neck area. Dr. Diwan will explain the details of your facelift procedure prior to your surgery. This will involve the tightening of your facial skin and lifting of the underlying facial tissue. Only the mid to lower region of the face will be addressed with this procedure. Patients looking to also correct signs of aging around their eyes or brow may benefit from combining their facelift surgery with eyelid surgery or a brow lift.

How Long is a Mini Facelift Recovery?

Eyelid Surgery

The eyes play a prominent role in facial expression and aesthetics. It is the eyes that often show the effects of time, age, and gravity before other parts of the face. The skin of the eyelids—being the thinnest of the body—may get wrinkled earlier. Aside from just wrinkling, the eyelid skin also becomes lax with the excess skin forming a fold on the upper eyelids or under the eyes. “Bags” form above and under the eyes and bulges appear, altering the natural, youthful contours of your eyes. Descent of the forehead can occur simultaneously causing crowding of the eyes. Dr. Diwan can perform a customized eyelid surgery procedure to restore your youthful appearance and improve the signs of aging.

How is eyelid surgery performed?

The excess skin, wrinkles, bags, or lowered brows make the eyes and face appear tired and older. Rejuvenation of the eyes can be achieved through cosmetic surgery treatments that address these problems, and help restore a more vibrant appearance. Blepharoplasty (eyelid surgery) is a surgical procedure that is designed to treat excess skin and bags around the eyes. Wrinkling of the skin can be treated using a combination of laser treatments and Botox®. Dr. Diwan may also recommend a brow lift, which restores the eyebrows to their original position by lifting the skin of the forehead and brow area. Depending on the patient’s needs, one or a combination of the above procedures may be needed to achieve optimal results. Dr. Diwan will recommend the best treatment plan for you based on your condition and desired outcome. Blepharoplasty and brow lift operations can be safely performed in-office.

How long is eyelid surgery recovery?

After your eyelid surgery procedure, Dr. Diwan will provide you with detailed care instructions to help you through the healing process. The typical recovery period for upper eyelid surgery is about one week; and two to four weeks for lower eyelid surgery.

When will I notice the final results of my eyelid surgery?

Continued improvement occurs over a few weeks before a final result is reached. This may vary based on the specific methods used for your eyelid surgery with Dr. Diwan. For a thorough evaluation of the problems that need correction, as well as the selection and discussion of appropriate treatments and planning, a consultation with Dr. Diwan is necessary.

Earlobe Repair

The tiny holes pierced in the earlobe for earrings can become elongated over time.  Factors such as the weight of heavy earrings worn daily; the pulling of  dangling earrings when they are caught on sweaters, hair brushes, or fingers; and the loss of elasticity of aging earlobe skin all work together to slowly pull on the small holes,  turning them into long slits or tears in your earlobes.  If you feel self-conscious or embarrassed by the look of your ears, and ill-fitting earrings, Dr. Diwan can reconstruct the earlobes to a  normal appearance.

How is earlobe repair performed?

Depending on the severity of your unique situation, the slits in your ears can extend all the way down  the earlobe, causing it to split and no longer be able to hold an earring. The split earlobe is a deformity that can be corrected in the office with a local anesthetic (numbing injection). After numbing the area, the edges of the split or long earlobe hole are removed and the fresh edges are sewn together. If desired,  the repaired earlobe is re-pierced in six weeks.  It is important to wear smaller, lighter earrings to prevent the same problem from recurring.

How long is earlobe repair recovery?

After reconstruction of the earlobe, there is some swelling and, possibly, bruising of the earlobes. Some minor pain or discomfort may occur immediately after surgery, but you should be able to resume your normal routine almost immediately. However, we do recommend avoiding higher levels of physical activity and avoid unnecessary contact with your ears for the first few days. Dr. Diwan will provide you with more detailed care instructions following your own personal earlobe repair procedure. Sutures are removed a week after the repair.

When will I notice the final results of my earlobe repair surgery?

You will notice an immediate improvement in the look of your earlobes once your surgery is complete. There may be some bruising and swelling for the first week as the skin heals. Most patients will enjoy the final results of their earlobe surgery within a few weeks. The earlobes can be re-pierced 6 weeks later.
