Basal cell cancer is the most common form of skin cancer, followed by squamous cell cancer. Melanoma is less common than either basal or squamous cell cancer, and it is the most aggressive of skin cancers. In addition to these three types, other less common forms of skin cancer can also occur.

Basal Cell Cancers
Basal cell cancers can have various appearances to the naked eye. Under a microscope, basal cell cancers also have different growth patterns – some being more aggressive than others. Different treatments are available for basal cell cancers. These include:
- Simple surgical removal followed by a repair of the skin
- Curettage & electrodesiccation (scraping & burning)
- Mohs Micrographic Surgery followed by reconstruction of the skin
- Treatment with Efudex cream
- Radiation therapy
All treatments take place at our office except radiation (which is performed at a hospital or outpatient radiation therapy facility). The treatment we recommend to you will depend on a number of characteristics of the skin cancer.
After considering the location, size, presence or absence of clear definition, failure of previous treatments, microscopic appearance, etc., we’ll determine the appropriate treatment.
Mohs Surgery
Mohs micrographic surgery provides the best cure rate for many skin cancers. We remove the cancer in-office with local anesthetic. The steps are as follows:
- Preparation: The surgery area is cleaned with antiseptics and numbed with local anesthesia for comfort.
- Cancer Removal: Dr. Diwan removes the cancer layer by layer, examining each layer under a microscope until only cancer-free tissue remains. This process is meticulous, involving waiting periods as the tissue is processed and she examines each layer.
- Reconstruction: After removing all cancerous cells, Dr. Diwan will reconstruct the site, often immediately following the removal, focusing on both functional and aesthetic outcomes.
- Recovery and Follow-Up: After surgery, patients receive care instructions and medications for pain management. We’ll schedule follow-up visits to monitor your healing, ensuring optimal recovery and cosmetic results.
This procedure is known for its high cure rate and minimized scarring, by preserving as much healthy tissue as possible.
In the majority of cases, the wound created in the skin after removing the skin cancer is evaluated and reconstructed the same day, with very gratifying cosmetic results.

Dr. Diwan will reconstruct the skin after skin cancer removal to restore your normal appearance and function. She has fellowship training in Procedural Dermatology, where she learned reconstructive techniques to repair wounds, paying close attention to aesthetic details. As a cosmetic surgeon, she applies refined cosmetic techniques to reconstructions, giving our patients the best possible outcome.

What Are the Signs of Basal Cell Cancer?

- A pearly or waxy bump
- A flat, flesh-colored, or brown scar-like lesion
- A bleeding or scabbing sore that heals and returns
These lesions appear most often on areas of the skin that are exposed to the sun, such as the face, head, and neck, and other sun-exposed parts of the body. If you notice any of these signs, it's essential to see a dermatologist or a specialized skin cancer surgeon for evaluation.
How Should I Prepare for My Skin Cancer Treatment Appointment?
To prepare for your appointment, please gather any previous medical records and biopsy results related to your skin condition and bring them along. Avoid applying lotions, creams, or makeup to the affected area on the day of your appointment. To minimize the risk of bleeding after surgery we recommend avoiding Aspirin and other anti-inflammatory meds such as Ibuprofen and Naproxen for at least one week before surgery. We also recommend avoiding alcohol and health supplements for at least 3 days before surgery.
It's also helpful to have a list of any medications you are currently taking and to note any allergies or past reactions to medications. We recommend wearing comfortable clothing and arranging for someone to drive you home if you're undergoing a procedure that might impair your ability to drive safely.
